We take nothing by conquest, Thank god. Zinn uses this statement to present how atrocious the conquest really was since there was a conquest. America wanted to hide it's evil ways behind the facade of saying that America acquire land always through peaceful purchases. On this chapter Zinn focuses on the Mexican American war and how it really was a conquest caused by Polk. In 1821, Mexico won it's independence in a revolutionary war against Spain. Mexico was a large country which included Texas, New Mexico, Utah, Nevada, Arizona and part of Colorado. In 183 Texas broke of from Mexico and declared itself † The lone star republic† and in 1845 the U. S Congress brought it into the union as a state.Later in spring of 1846, all that was needed was a military incident to begin the war Polk wanted. It happened after General Taylor's quartermaster, Colonel Cr oss isappeared. His body was found eleven days later. It was assumed that the Mexicans had killed him. A lot of the Anti-Slavery congressmen voted against all war measures. Seeing the mexican campaign as a means of extending the southern. Slave territory Joshua Giddings explains his vote against supplying men and arms for the war.To Howard Zinn looks like Polk was the only one that wanted California to be part of his nation. Post-Traumatic Stress Disorder symptoms Essay

I. Importance of Study Found in the Journal of Advanced Nursing, a group of Norwegian psychologists conducted an observational longitudinal study to discover whether or not there is a relationship because Health Related Quality of Life (HRQoL), and Post-Traumatic Stress Disorder symptoms (PTSD). Mette Senneseth, Kjersti Alsaker & Gerd Karin Natvig, conducted this study. According to the journal, the aim of the study was to examine HRQoL and PTSD symptoms in the people that attend Accident and Emergency departments (A&E), due to the fact that they suffer from some type of psychosocial crisis (Alasker et al. 2011, 403). Hereafter, this is referred to as Research 1. The study done in Research 1 revealed that there has been an interest in researching one’s quality of life, and how it correlates with one’s health. The term HRQoL refers to the â€Å"effects of health, illness and treatment on QoL† (Alasker et al. 2011, 403). There have been a number of studies that suggest that traumatic lif e events, including, but not limited to, sexual assault and military combat, have a negative effect on HRQoL. Additionally, poor HRQoL is not only associated with PTSD symptoms, but they are related to differences and modification in HRQoL, and there is a negative correlation between the two (Alasker et al. 2011, 403). The researchers hypothesized that people who seek help at A&E, suffering from such psychosocial calamites, have lower HRQoL than the general population of Norway at the time of the study, and that the participants will have improved HRQoL after two months (Alasker et al. 2011, 403). In addition, a second hypothesis predicts that participants who have high levels of PTSD symptoms at the time of the study will have reduced symptoms after the conclusion of the study at two months. Lastly, the third hypothesis wraps the study up by predicating that high levels of PTSD symptoms are associated to low HRQoL scores in a follow-up experiment (Alasker et al., 2011, 403). Found in the Cyber-psychology, Behavior and Social Networking Journal, a group of researchers conducted a study to compare the effects of VR-graded exposure therapy (VR-GET) versus treatment as usual (TAU) on people suffering from combat related PTSD (McLay et al. 2011, 223). Robert Mclay, Dennis Wood, Jennifer Webb-Murphy, James Spira, Mark Miederholf, Jeffery Pyne and Brenda  Wiederhold conducted this study. Hereafter, this is referred to as Research 2. The doctors responsible for conducting this study found that there has only been one â€Å"randomized, controlled proof-of-concept† study that was specifically designed for Active Duty Service Members suffering from PTSD (McLay et al. 2011, 223). Also, the doctors wanted to extend the â€Å"already found research gathered† from victi ms of PTSD, and take the study one step further. The researchers of Research 2 hypothesized that â€Å"patients with combat-related PTSD would be more likely to experience clinically significant improvements in VR-GET than treatment as usual† (McLay et al. 2011, 224). VR-GET is a virtual reality stimulation that combines graded virtual reality exposure with â€Å"physiologic monitoring and skills training† (McLay et al. 2011, 224). Also, VR-GET promotes engagement with, rather than escaping the events and experiences that prompt a traumatic episode. II. METHODS In regards to Research 1, the participants in the study had to meet three criteria before they were able to participate. During the recruitment period, the researchers found participants who were attending the A&E due to a â€Å"psychosocial crisis and who consulted a psychiatric nurse,† were 18 years of age or older, and those who were able to both read and understand Norwegian (Alasker et al. 2011, 404). A total of 113 were asked to participate in the study. Of those 113 people selected, 99 of the people participated in the baseline study, and 41 participated in both the baseline study, and the follow-up study. Aforementioned, this was an observational longitudinal study, where participants were observed repeatedly over a long period of time. There were two types of instruments that were used for the research in Research 1. The first instrument that was used was a SF-36 Health Survey, which is a â€Å"36 item self-report questionnaire that assesses eight domains of physical and MH ranging from 1-100† (Alasker et al. 2011, 404). In this survey, the higher score reveals the best HRQoL, and the lower score reveals the poorest HRQoL (Alasker et al. 2011, 404). The second instrument that was used for this study was the Post-traumatic Symptom Scale (PTSS-10), which is a ten item self-report questionnaire that â€Å"assesses the presence  and intensity of symptoms† (Alasker et al. 2011, 404). On the PTSS-10, scores range from 10 to 70, and a score of 35 or greater results in a PTSD diagnosis (Alasker et al. 2011, 404). In addition to the two questionnaires, participants were encouraged to go to consolations at the A&E. The number of consultations that each participant attended varies throughout the study. In regards to Research 2, â€Å"participants for the study were all Active Duty Service Members who had been diagnosed by a military mental health professional as having PTSD† related to military combat (McLay et al. 2011, 224). At the end of the baseline assessment, those who were qualified to participate in he study were asked to draw a piece of paper out of an envelop, making the selection of treatment completely random, and giving the participants an equal chance of choosing either treatment. There were a total of twenty people that participated in this study; ten assigned to the VR-GET and another ten assigned to TAU (McLay et al. 2011, 225). The two methods of Research 2 consisted of the VR-GET and the TAU. The participants that were assigned for the VR-GET had a sequence of sessions that consisted of different activities. First, the therapist would meet with the participant and discuss their trauma history. In the second session, participants were asked to reveal their more traumatic stories of their military combat and tours. In later sessions, the participant used the virtual reality helmet to relive their most traumatic events, based off of the information that was gathered about them in the earlier sessions. In each session, the participants were observed on their ability to face their fears and anxieties (McLay et al. 2011, 225). â€Å"Participants assigned to TAU could receive any of the regular services available to them at the NMCSD and NHCP. These two facilities offer full spectrum of PTSD treatment, including, but not limited to, cognitive processing therapy, prolonged exposure, and group therapy† (McLa y et al. 2011, 225). III. ANALYSIS/RESULTS Analysis The researchers for Research 1 used the two self-report questionnaires to gather their data. In addition, the researchers processed the data using  statistical analysis with the help of SPSS16 Processor for Windows (Oslo, Norway). The researcher for this study wants to compare the results of the two surveys with the general population of Norway, and to succeed with that, they used SF-36 data through the Norwegian Coordinated Living Conditions Survey from 2002, consisting of 5131 people (Alasker et al. 2011, 405). In Research 2, it was found that participants in the baseline study had lower norm-based scores in all eight HRQoL domains in comparison to the general population of Norway. In the follow-up study, two months later, the â€Å"participants still had lower norm-based scores than the general population of Norway, but participants had improved their HRQoL in five of the eight domains from the baseline study to the follow-up study† (Alasker et al. 2011, 406). In regard to the PTSS-10 questionnaire in Research 1, among the participants in the baseline study, 79% of them had a PTSS-10 score that was 35, expressing high levels of PTSD symptoms (Alasker et al. 2011, 406). At the follow-up, â€Å"59% of the participants had a PTSS-10 score that was 35, which shows high levels of PTSD symptoms, which can indicate a risk of developing PTSD† (Alasker et al. 2011, 406). The researchers for Research 2 aimed to identify which of the VR-GET or TAU would yield a greater percentage of participants with a â€Å"clinically meaningful reduction in PTSD† (McLay et al. 2011, 225). The researcher succeeded in deepening their research by investigating the â€Å"difference in CAPS scores at an initial assessment and then at the post-treatment assessment in VR-GET versus TAU† (McLay et al. 2011, 226). The CAPS is a â€Å"rating scale for PTSD that corresponds with the 17 symptoms of PTSD† (McLay et al. 2011, 226). Participants were classified according to whether or not they had a 30% of larger reduction of their PTSD based on the results of their CAPS. ii. Analysis Results Taking a look at Research 1, and the question concerning whether or not there is a link between PTSD symptoms and HRQoL, researchers looked at the differences in SF-36 scores between PTSS-10 subgroups in a follow-up study (Alasker et al. 2011, 406). â€Å"The PTSS-10 high scoring and low scoring subgroups at the 2-month follow-up differed in all eight of the HRQoL domains in the follow up study† (Alasker et al. 2011, 406). In addition, the PTSS-10 low scoring participants had improved HRQoL in six out of the eight domains (Alasker et al. 2011, 407). In regards to Research 2, all ten of the participants assigned to the VR-GET were assessed with the CAPS at the post-assessment. Seven out of the ten participants showed an improvement of 30% or more on the CAPS. On the other hand, out of the ten participants that were assigned to the TAU, one did not complete a CAPS assessment. Nevertheless, one out of the nine returning participants receiving the TAU revealed more than a 30% improvements on the CAPS (McLay et al. 2011, 226). â€Å"There was no significant difference between VR-GET and TAU average CAPS scores both before and after the treatments, but there was indeed a significant difference in the average CAPS score over the course of the entire treatment† (McLay et al. 2011, 226). IV. DISCUSSION Results Summary Concerning Research 1’s first hypothesis, participants of the study reported lower HRQoL compared to the general population of Norway in all eight HRQoL domains (Alasker et al. 2011, 408). In regards to the second hypothesis, participants in Research 1 reported high levels of PTSD symptoms at the time of the baseline experiment. Furthermore, PTSS-10 scores did improve from the time of the baseline experiment to the follow-up. The results show that PTSD symptoms decrease for people suffering from a psychosocial crisis in the 2 months after attending the A&E (Alasker et al. 2011, 408). Lastly, concerning the questions if there is a link between the level of PTSD symptoms and HRQoL in the follow-up study, the researchers found that a â€Å"high level of PTSD symptoms after a two month period were linked to lower HRQoL† (Alasker et al. 2011, 408). Results for Research 2 Strengths and Weaknesses In regards to Research 1, there are many strengths and lurking variables that should be pointed out. One strength of the study was that they  progress of the study was completely up to the participant. The participant had the option as to how may consultations they wanted to attend, and the varied decisions led to different results. Another strength of the study was that they used instruments that were both appropriate to the study, and reliable. On the contrary, there were several confounding variables in Research 1 that must be acknowledged. First, there is no evidence that anyone in this trial was on any sort of anti-depressant or medication that would affect his or her scores on the SF-36 and the PTSS-10. Also, there are so many different traumatic events that would cause someone to show symptoms of PTSD. The study should limit its participants to those who experienced similar types of trauma. As for Research 2, there were also strengths and weaknesses to the study. As for its strengths, the doctors did a good job through their selections process to select a small group of people who were highly qualified for this study. Also, the exam that was used to measure the severity of the participants PTSD was based off of seventeen symptoms of PTSD, whereas the instrument used in Research 1 (PTSS-10) only focused on ten symptoms of PTSD. Lastly, the ten-week span of the study was appropriate in yielding results. Just like any other study, there are confounding variables that need to be identified for Research 2. Although the goal was to get the participants in for a reassessment at the conclusion of the study (ten weeks), reassessment for some did not occur until as far as 36 weeks. With that being said, participants have plenty of time to have a relapse, or more time in combat that can result in more severe PTSD. iii. Future Directions The researchers of Research 1 suggest that a â€Å"randomized control trial with a control group is needed to investigate the effect of the psychosocial interactions that are given to this group† (Alasker et al. 2011, 407). In addition, they suggest that it is crucial to get more, information about the long term effects of acute crisis intervention on PTSD symptoms and HRQoL â€Å"given by psychiatric nurses to participants† (Alasker et al. 2011, 410). The researchers of Research 2 also have some suggestions for further directions of this research. It was stated that other studies on the topic  of virtual reality therapy on PTSD victims, improvements in symptoms aren’t visible until sometimes three months after treatment. For future studies there needs to be a longer wait time for the follow-ups so that there can be the most accurate results. In addition, the journal states that careful monitoring of the participants is also something that needs to be done more thorough in the future because a participants overall health, aside from PTSD, must be taken into consideration when the researchers are making observations and conclusions about the participants (McLay et al. 2011, 226). A Tale of Two Cities by Charles Dickens Essay Throughout A Tale of Two Cities, Charles Dickens tells the story of several characters, all of who are developed continuously throughout the chapters. Several of the characters are greatly developed, some showing unpredictable sides to their personalities. Sydney Carton is a prime example of such a character, his delicate personality revealed when he declares his love for Lucie. He is not the only character in love with Lucie, however. Throughout Book Two, it is revealed that Stryver and Charles Darnay are also coveting Lucie. Darnay and Carton both announce their love for her, however Carton is the only one who goes directly to Lucie to tell her. In this passage, Carton is asking Lucie to not forget him and to remember that even when she is married and with kids, he will always be there for her to keep her happy. Carton’s declaration of his love for Lucie not only reveals his love and foreshadows his selfless, noble act in the later chapters, but in addition, Dickens’ use of language reveals that Sydney is no longer the â€Å"jackal,† but that he is in fact much more than that – a sensitive man with deep emotions. Dickens’ use of language in this passage brings to light the sincerity in Sydney Carton, changing the mood of the reader towards him, and the words that Dickens uses to make Carton express himself allows Carton to truly show his desire for Lucie. When Carton says, â€Å"for you, and for any dear to you, I would do anything,† Dickens is emphasizing the fact that Carton is dedicated to her. Throughout the passage, he continuously repeats phrases including the word you, such as when he states â€Å"I would embrace any sacrifice for you and for those dear to you.† As a result of emphasizing Carton’s love for Lucie, Dickens creates an immense feeling of sympathy for the reader, as the dedication for Lucie is expressed so eloquently. The fact that Carton accepts this unrequited love by saying â€Å"I will relieve you of a visitor with whom I well know you have nothing in unison, and between whom and you there is an impassable space† creates even more comp assion for Carton because even though it is known that Lucie does not reciprocate his feelings, it is even more painful to watch as he simply walks away from the woman he loves. In addition to uncovering Carton’s true love for Lucie, Dickens uses irony to show the eloquent Carton that contrasts with the drunken Carton of the previous chapters. The chapter to which this passage belongs to is titled â€Å"The Fellow of No Delicacy,† which is ironic because although perhaps Carton was not delicate in the preceding chapters, in this passage he is nothing but eloquent and delicate. He remarks â€Å"try to hold me in your mind, at some quiet times, as ardent and sincere in this one thing,† a great example of his eloquence, which once again causes the reader to develop sympathy for him, since the language he is using so differs from his previous drunken language à ¯Ã‚ ¨203). Carton’s eloquence really shines through in this passage, even though it contrasts with the chapter title and Dickens creates sympathy and tenderness towards Sydney Carton. Because Sydney Carton is put down, not only by others but by himself as well, throughout the entire first half of the second book, when he declares his love for Lucie in such an unselfish and loving manner, it changes how Carton is viewed. Since Stryver takes all the credit for being successful even though Carton does all the work, it is written, â€Å"†¦although Sydney Carton would never be a lion, he was an amazingly good jackal† à ¯Ã‚ ¨116). Carton even describes himself to Darnay as â€Å"I am a disappointed drudge, sir I care for no man on earth, and no man on earth cares for me,† exhibiting Carton’s own self-pity and sadness for the world à ¯Ã‚ ¨113). However, with this passage, Sydney Carton causes any reader to fully disregard any judgments made for his character. Dickens previously portrayed him as this unfortunate man without anything to live for, but now it is revealed that Carton does indeed have someone to live for – Lucie and her happi ness. This passage causes all previous opinions about Sydney Carton to be discarded, as now Sydney Carton is no longer the jackal, but he has turned into the lion. In addition to revealing the compassionate and eloquent side of Sydney Carton, this passage is also a moment of foreshadowing for Carton’s noble and unselfish act of sacrificing himself for Lucie’s happiness. What may have seemed to be just talk about how much he loved Lucie turned out to be very, very real. In this passage, Carton declares to Lucie, â€Å"†¦think now and then that there is a man who would give his life, to keep a life you love beside you† à ¯Ã‚ ¨204). In his last moments, Carton visualizes Lucie feeling that â€Å"each was not more honoured and held sacred in the other’s soul, than I was in the souls of both,† referring to the respect and recognition that both Darnay and Lucie give to Carton à ¯Ã‚ ¨500). In this passage, Carton sets up his feelings for her, perhaps knowing that someday he would have the chance to occupy a special place in Lucie’s heart for a noble, heroic act. His â€Å"sacrifice for you and for thos e dear to you† embodies the Charles Dickens’ theme of love overpowering everything. With love, Carton was able to have the strength to sacrifice his life for the happiness of one person he cared about. With love, not jealousy, he was able to ask Darnay, â€Å"I wish we might be friends† à ¯Ã‚ ¨275). While Lucie does not reciprocate Carton’s love, she does defend him in front of Darnay, declaring â€Å"I would ask you†¦to be very generous with him always†¦I would ask you to believe that he has a heart he very, very seldom reveals, and that there are deep wounds in it† à ¯Ã‚ ¨278). By Carton declaring his love for Lucie and being wholly devoted to her, he is not only able to give Lucie a complete family, safe and protected in England, but he is also able to say that â€Å"it is a far, far better thing that I do, than I have ever done; it is a far, far better rest that I go to, than I have ever known,† clearly displaying that through his death, he is gaining all the love and admiration he never had before à ¯Ã‚ ¨500). Sydney Carton is my favorite character of the entire novel. His noble act despite the unrequited love between him and Lucie is such an honorable action, and the fact that he kept true to his word about giving his life â€Å"to keep a life you love beside you† makes him all the more noble à ¯Ã‚ ¨204). I thought that this passage was really a turning point for Sydney Carton’s character, since up to that point he had been very reserved and inarticulate, but when he went to speak to Lucie he became this eloquent speaker, driven by a strong love. From then on, we continue to see Carton’s character develop, creatively seeking out the apothecary for the substances in order for the plan to work. He acted without a moment’s hesitation, and that â€Å"[his face] was the peacefullest man’s face ever beheld there† shows that he had absolutely no regrets about his sacrifice à ¯Ã‚ ¨4à ¯Ã‚ ¹Ãƒ ¯Ã‚ ¹). To be that calm when he is about to be the newest victim of the Guillotine is a daunting task in itself, but to have someone who would sacrifice his or her life just for someone else’s happiness is beyond imaginable. Therefore, this passage sets the stage for Carton’s ultimate sacrifice, making Carton no longer a â€Å"disappointed drudge,† but a noble hero who goes from being a friend to Darnay and the Manettes to becoming their savior. There is no greater sacrifice that Carton could have made, and for it all to be because he loved Lucie unconditionally is a pretty incredible thing.

Resulting of the increasing of the mortality rate CPR and AED training are important for nursing staff and is important as the nurses often are the first witness for the victim with cardiac arrest in hospital setting. The outcome of teaching approach is necessary to improve the knowledge and skill of the nurses and other medical team that increase the survival rate. Aim The aim of this literature review was to describe teaching approaches for cardiopulmonary resuscitation education in relation to acquired skills and knowledge in the use of an automated external defibrillation in a cardiac arrest situation in a clinical setting. Method The authors have selected a literature review for this study and key word that used are Cardiopulmonary resuscitation and education, nursing, continuing, cardiopulmonary resuscitation, nursing skill and education and cardiopulmonary resuscitation. Papers published two month ago since 04-2011 from many sources, such as books and Pubmed database including 10 years period. Those published paper in English language which covers strategies to improve the knowledge and skills of the nurses and other medical staff in cardiopulmonary resuscitation and automated external defibrillation using. In addition manual research has been used to get more information. Result There are many different teaching approaches can be used in cardiopulmonary resuscitation and automated external defibrillation training such as ; Digital Video Disc without manikin, DVD with manikin with scenario, peer tuition and instructor facilitated methods which the best method. The quality of education in cardiopulmonary resuscitation and automated external defibrillation training can be affected by the type of teaching approach which has been used. The education and refreshment of cardiopulmonary resuscitation and automated external defibrillation training can enhance skill and knowledge of nursing staff and give self confidence during resuscitation time and in using automated external defibrillation, which can lead to increase the survival rate and improve the quality of patients life. Conclusion Nurses in hospital setting who seldom encounter cardiac arrest, should received automated external defibrillation and cardiopulmonary resuscitation training to avoid time delays in resuscitation. Staff should be evaluated by using manikin with feedback system and expert instructors to ensure the quality f chest compression are satisfied at the time of training. The availability of resuscitation training equipment in unit and ward can enhance the nursing staff for self education and refresh their skill and knowledge. INTRODUCTION Each year in United States of America there are 325,000 victims of sudden cardiac arrest. The mortality rates due to cardiac arrest are frequently observed in most of emergency situation, which is most often the result of ventricular tachycardia (VT) according to Mosesso, Shapiro, Stein, Burkett, Wang (2009). Moreover, cardiovascular attacks cause one third of all deaths in the world today (Lindstrand, Bergstrà ¶m, Rosling, Rubenson, Stenson Tylleskà ¤r, 2010). The nursing staffs are often the first witness for cardiac arrest in a hospital setting (Dwyer, Mosel, 2002; Gombotz, Weh, Mitterndorfer Rehak, 2006). Moreover, the nurses are more efficient in training of automated external defibrillation (AED) as stated by Xanthos et al.2009. Bardy et al (2008) point out that due to the effectiveness of automated external defibrillation, it becomes recommended in public places such as; public places are: airports, schools, restaurants, planes, and crowded places. Furthermore they recommend that the AED machine is red in color with instructions for use. In spite of the effectiveness of AED in cardiac patients, a better understanding and knowledge about the use of AEDs is required (Hancock, Roebuck, Farrer Campbell, 2006; Martinez-Rubio, et al.2003).Therefore, the authors` see a need to explore how Cardio-Pulmonary Resuscitation (CPR) education can enhance and improve nursing skills and knowledge in the use of AED in order to reduce mortality rate in cardiac patients. BACKGROUND Automated external defibrillation www.priceit.in AED is an electronic devise which has two paddles; sternum and apex which can help to deliver the shock to the heart of the patient. This machine has been created and upgraded to convert the VT and Ventricular fibrillation (VF) to sinus normal heart rhythms, which are the most emergency situation that needs immediate medical intervention (Anas, Lee, Hasan, 2010). However, using this machine needs medical education and knowledge as stated by (Ammirati et al.2011).As mentioned above Brown, Rittenberger, Ammon, Harrington Guyette (2010) stated AED can have affect only in shockable rhythms like VF. Types of automated external defibrillation There are two types of AED: Monophesic which gives (360 joules) and the secound type is Biphasic which gives 150-200 joules (Sraj, 2005). VT clinical association VT occurs with Coronary Artery Disease (CAD), cardiomyopathy, significant electrolyte imbalance, mitral valve prolapse, long QT syndrome, some medicine toxicity like digitalis and central nervous system disease. Furthermore, this type of dysrhythmia can be notice in non cardiac patients (Lewis et al, 2007). VT clinical significance Patient with VT may be found with pulse or pulseless and VT if persistent can lead to decrease in Carbon monoxide (CO) due to decreasing of ventricular filling times and loss of atrial contraction. All this finding can lead to several complications which include; hypotension, pulmonary edema, cerebral blood flow will decrease and cardiopulmonary arrest (Lewis et al, 2007). VT treatment This type of dysrhythmia can be treated simply if the causes identified like; electrolyte imbalance and ischemia. If the VT is monomorphic (patient has appearance of all the beats match each other in each lead of a surface electrocardiogram) and patient with pulse, Procaimade, Sotalol, Amiodarone or Lidocaine Intravenous injection (IV) can be used. If the patient in polymorphic (has beat-to-beat variations in morphology) VT with prolonged baseline QT interval Magnesium, Isoprterenol, Lidocaine Intravenous injection can be administer. If the patient has no pulse it must be taken in high priority level emergency case, in this condition CPR and defibrillation are the first line if treatment (Lewis et al, 2007). Ventricular fibrillation (VF) Ventricular fibrillation is described as a heart rhythm witha severe derangement of the rhythm characterized on electrocardiogram (ECG) by irregular undulation of varying shapes and amplitude (Lewis et al., 2007, p. 855). VF clinical association VF can occur in acute myocardial infarction (MI), myocardial ischemic disease like Coronary Artery Disease and cardiomyopathy. Furthermore this type of abnormal heart rhythm can be associated with coronary reperfusion after fibrinolytic therapy. Other VF clinical associations are like electric shock, hypekalemia, drug toxicity and acidosis (Lewis et al, 2007). VF clinical significance Patient with VF can be found unconscious, pulseless and apneic condition and if this type of dysrhythmia not treated immediately and rapidly patient will die (Lewis et al, 2007). VF treatment VF is life-threatening condition and the best treatment for this dysrhythmia is the CPR and Advance Cardiovascular Life Support (ACLS) with using defibrillation immediately if available (Lewis et al, 2007). Cardiopulmonary resuscitation (CPR) Cardiopulmonary resuscitation is a combination of oral resuscitation (mouth-to-mouth breathing), which supplies oxygen to the lungs, and external cardiac massage (chest compression), which is intended to normalize cardiac function and blood circulation(Kozier, Erb, Snyder Berman, 2008, P.1419). Furthermore, these precautions are very important in keeping the operator and the patient in a safe and secure environment. These precautions are; Unresponsiveness The rescuer should check the patients level of consciousness, pulse and any signs of life like moving or coughing. This step is very important step to avoid any problems or misusing of AED. Wet chest Due to water and sweat AED might not be very effective. So In order to get defibrillations effectiveness use the piece of cloth which is kept in the AED kit to dry the chest Moving vehicle Using AED devise needs special care especially during the transportation of the patient. In case of using moving vehicle it may affect AEDs analyzing the heart rhythm which can lead to wrong analysis. Flammable materials Try to avoid using any flammable materials like alcohol and stop any flammable gases like oxygen .using these flammable materials can make fire due to the expected spark from the shock. Chest patches Be close to the patient and scan for any nitroglycerine patches and remove it right away before attaching the AEDs paddles. These patches can make burn to the patients chest. Metal surface Avoid using the metal or any electric conductive surface because the ability of transmitting the shock to the rescuers or any persons nearby. Pacemaker The rescuer should look to the patients chest closely and identify if this patient using pacemaker especially if its implanted one. Try to attach the paddles one to one and have inches distance from the pacemaker to avoid destroying the pacemakers batteries. EHR Implementation Background The implementation of electronic health records (EHR) continues to make an impact on nursing and patient care throughout the country. As a part of the American Recovery and Reinvestment Act of 2009, all public and private healthcare providers were required to implement electronic health records in their facilities by January 1, 2014. By demonstrating â€Å"meaningful use† of the electronic medical record, facilities are able to maintain Medicaid and Medicare reimbursement levels. Providers who show that they are meeting the â€Å"meaningful use† criteria during EHR use will receive an incentive payment from Medicare and Medicaid. â€Å"Meaningful use† is â€Å"using certified technology in EHR implementation to improve quality, safety, efficiency, and reduce health disparities; engage patients and families; improve care coordination; and maintain privacy and security of patient health information† (Centers for Medicare & Medicaid Services, 2013). The federal government has encouraged EHR use in hopes that it will significantly improve patient care. There is the intent that electronic health records will allow any provider access to important patient health information no matter where the patient is, while â€Å"creating a comprehensive national electronic health information network that leads to a reduction in the duplication of tests, an improvement in the cost-effectiveness of interventions, and the ability to compile a comprehensive patient history† (McBride, Delaney, Tietze, 2012). Mechanisms and Processes of the Internet Electronic commerce can be defined as the conduct of commerce in goods and services, with the assistance of telecommunications and telecommunications-based tools. Here, the term telecommunications can mean any existing telecommunication networks such as Public Switched Telephone Network, Integrated Services Digital Network, or even Wireless Networks. However the major and the most important telecommunication network of E-commerce is the Internet. The great possibility and potential of E-commerce has been realized with the development of the Internet. In this paper I will discuss the mechanisms and process of the Internet. The Basic Process When we surf the web, we type any URL in to our browser, and the desired page pop up on our screen. At the most basic level possible, the following diagram shows what happened: Our browser formed a connection to a web server, requested a page and received it. Here are the basic steps that occurred behind the scenes:  · The browser broke the URL into 3 parts: 1) The protocol ("http"), 2) the server name ("www.howstuffworks.com"), and 3) the file name ("web-server.htm").  · The browser communicated with a name server to translate the server name "www.howstuffworks.com" into an IP Address that it can use to connect to the server machine.  · The browser then formed a connection to the server at that IP address on port 80.  · Following the HTTP protocol, the browser sent a GET request to the server asking for the file "web-server.htm".  · The server returned the HTML text for the web page to the browser. Outlines of the Theory of Choice There are two distinctions of orientation or of intention of a subject toward any phenomenon: "to" or "from" it, attraction or repulsion, acceptance or rejection. The +/- acceptability or pleasantness/unpleasantness of a phenomenon to a subject is the term indicating his or her +/- orientation to the perceived phenomenon. There are six components of the stream of human consciousness: contact senses (smell, taste, tactile senses), distant senses (auditory, visual) and emotions. Only four of them (the three contact senses and emotions) possess their own acceptability or pleasantness. Pleasantness of Condition of a Subject (PCS) is a sum or an integral of acceptabilities of these four components. "Happiness" is the upper limit of the maximization of PCS; a subject is constantly striving to maximize PCS or to reach for happiness. An attitude of a subject to a phenomenon in the center of his or her attention is determined by the synchronous PCS. Belief/disbelief is a verbalized positive/n egative attitude. Desire of a phenomenon x is a change of PCS (PCSx) created by the act of perceiving/imagining the phenomenon; the strength of desire is the magnitude of this change. Desire of a phenomenon characterizes power of the PCS maximization possessed by this phenomenon. Need is a periodic desire; the desire correspondent to need is a concrete form of existence of this need. Choice is determined by comparative strength of the desirabilities of the competing elements of choice; it includes choice of the phenomena to perceive or attend. The attention of a subject toward a perceived phenomenon x is proportional to the strength of its desirability: ATTx=kPCSx; the distribution of attention is a function of the desirabilities of phenomena perceived at that time. Will is an ability of the subject to influence the balance of desirabilities of elements of the subject's choice in the predetermined way. The nature of the will's effort is a self-inducement of suitable emotions through activation of memories by the concentration of the subject's attention to them. I. The Universal Intentional or Orientational Quality of "Acceptibility" Process of choice is vitally important for animals including humans because they are open, active, and limited systems. The term orientation is used in this work in two meanings corresponding to two main aspects of choice ¡Xan appraisal of the elements of choice one by one, and their comparison resulting in the choice being made. An appraisal of a singled out element of choice is also an act of orientation of a subject to it that can be 'positive' or 'negative', 'toward' or 'away' from it.
